How to File a Claim

Filing a claim with Aflac is simple. You can file online, through the mobile app, or by fax/mail.

Log In to MyAflac

Visit aflac.com or open the MyAflac app. Register or log in using your policy/certificate number, Member ID, or Social Security number.

Select "Submit a New Claim"

Choose the policy you want to file against and select the type of care (e.g., routine medical, accident, illness).

Upload Documentation

Attach any required supporting documents such as medical bills, itemized statements, or doctor’s notes.

Sign & Submit

Electronically sign your claim and submit it. Track status through your online message center or via email updates.
